MEDICAL CHRONOLOGY SERVICES

Chronologies and summaries, drafted from the record — every entry cited to its page.

Medical chronology and summary services generate the deliverables you'd otherwise send to a review service — a date-ordered chronology and a case-ready summary — in minutes from the file you upload. Every entry is drafted from the record itself, and every line is linked back to its source page for verification.

The same deliverable. Not the same week of waiting.

A chronology from a review service means intake, abstraction, a spot-check pass, and an invoice by the page (plus a rush fee when the deadline moves). Keep the work with you instead. Upload the file, and the chronology and summary come back in minutes, every entry already pointing at its source page.

How it works

Three steps between the upload and the export.

01

Upload the records you already have

The file as it arrived: mixed formats, duplicates, out-of-order pages. In the demo case, 342 pages across 2 packets, with 11 duplicate pages removed before anything is drafted.

02

The AI drafts both deliverables, every line cited

The chronology assembles in date order and the summary drafts alongside it, each entry and each sentence linked to its source page. Suspect pages are quarantined, not quietly used.

03

You review, verify, and export

Check any entry against its source with a click, then export a hyperlinked DOCX or PDF. The citation links stay live in the document you send.

Built to be checked

A timeline that shows its work, entry by entry.

A chronology is only as good as the answer to one question: where did that come from? Here, every entry carries its source page, so the person who checks the timeline (you, a colleague, or opposing counsel) can trace any date back to the record in one click.

In the demo case, a prior-injury page (p.140) belonged to a different patient. It was quarantined and logged, never woven into the timeline. When someone asks how the chronology was produced, the audit trail answers for you.

HIPAA, under a signed BAA

Every file is handled under our Business Associate Agreement, from the first byte.

Never trains a model

Your records are never used to train any AI model — ours or anyone else's.

Every line cited

If we can't cite it, we don't say it. Every sentence links to its source page.

Deleted after delivery

Files are deleted 30 days after delivery, with a full audit log of every access.

Powered by the platform

Four capabilities behind every timeline.

The chronology and summary are two deliverables of the same platform that reads, sorts, and cites the whole record.

Medical chronology

The timeline that builds itself: every visit, image, and gap in date order, each entry synced to its source page.

IN ACTION · 342 pp → ER 2/14 p.4 · MRI 3/02 p.18 · surgery 4/18 p.61

Medical summary reports

The case-ready narrative built from the same cited record: your template, your letterhead, every sentence sourced.

IN ACTION · 342 pp → case-ready summary · cited 100%

Smart lists

Providers, medications, diagnoses, and gaps pulled into structured lists, each row carrying its own citation.

IN ACTION · 3 events · 7 visits traced → every row cited to its page

Supplemental record review

Late-arriving records merge into the same case: deduplicated, new facts summarized, conflicts flagged for review.

IN ACTION · packet 2 merged → 11 pp deduplicated · p.140 quarantined
FAQ

Chronologies and summaries, answered.

Three views of the same cited timeline: date-ordered (every event in sequence), provider-grouped (each facility's thread in one place), and topic views (one condition or body part traced across the whole file). Switch between them without rebuilding anything; every entry keeps its link to the source page.

Upload the new packet to the same case. It is deduplicated against the existing file, the new facts are summarized on their own so you can see exactly what changed, and anything that conflicts with the earlier record is flagged for your review. The chronology updates in place — no starting over.

Yes. Chronologies and summaries export as hyperlinked DOCX: the citation links stay live in the document, so anyone reading it can click from an entry straight back to the source page it cites. PDF export is available too.

No. MedRecords AI does not retrieve records from providers or facilities. You bring the records you already have — review starts in minutes from upload. Retrieval vendors take days; you can keep yours and still cut the review to minutes.

Files are deleted 30 days after delivery, and every access in between is written to a full audit log you can export. Your records are handled under a signed BAA and are never used to train any AI model.

A medical summary condenses the most clinically relevant facts from a file. A medical chronology arranges every event in date order so you can see treatment progression, causation issues, and gaps in care at a glance. Medrecords AI generates both, plus an Extractive mode with zero inference when you need citation-only output for exhibits.

The chronology you'd wait a week for, before your coffee cools.

Every entry cited. Every line linked to its page.
